Health Forms

The school board approved a new policy change for all health forms to be due on the first day of school instead of October 15th, and will be collected at registration in August!
  • The only exception is if you consent to the dental clinic!
  • Dental clinic is held in the spring but is only available to those who qualify for free/reduced lunch or on the medical card!
  • If you do not turn in a physical or up to date immunizations for the required grades by the first day of school your child may be excluded from school until this is turned in.
  • If you do not turn in a dental or vision exam form, your child's report card will be held until this is turned in.

  • the use of whole grains
  • require that all milk be 1% or nonfat.
